ACTIVITY WAIVER TERMS

Activities Registration Waiver Terms

By registering your child for any programs or activities, you are agreeing that you will abide by our Attendance Policy and hereby give your child permission to attend that program or activity. 

Registration is first come, first served. 

By registering my child(ren) for any activities or programs, permission is granted to the Borough of Totowa and The Borough of Totowa Public Library to use any photographs in any Borough of Totowa Public Library promotional material (e.g., brochures, website, newspaper press). I waive any right to inspect or approve any use of the photographs. On behalf of myself and all other persons or entities, I release and forever discharge the Borough of Totowa and its employees, officials, and agents from any and all claims, demands, or causes of action of any kind or nature arising from the use of the photographs under this release. I have read this document and understand its contents. 

YOGA / MEDITATION / MINDFUL CLASS REGISTRATION, RELEASE & WAIVER AGREEMENT

PLEASE LET THE INSTRUCTOR KNOW OF ANY MEDICAL OR HEALTH CONCERNS OR INSTRUCTIONS PRIOR TO PARTICIPATING.

 

EMERGENCY TREATMENT, RELEASE & WAIVER AGREEMENT:

 

· I am aware of the hazards of the activity/sport and the risk of injury in this athletic program; I certify that I am in good physical condition and am able to safely participate in this physical activity/sport.

 

· I assume all risks and hazards incidental to such participation, including transportation to and from activities, and do hereby waive, release, indemnify and agree to hold harmless the Borough of Totowa Public Library and Borough of Totowa, Volunteers and staff, team or league sponsors from all liability for any and all loss or damage, and any claim arising out of injury to myself or property damage that might occur, whether caused by negligence of the Library of Municipality, agents or employees, during participation.

 

· In the case of emergency, I hereby give my permission to the medical personnel selected by the Library staff, to act as my agent to hospitalize, secure proper treatment for, to order x-rays, routine test, or other medical treatment for myself.

COME INTO THE LIBRARY TODAY TO REGISTER AND BEGIN READING!

Numerous studies estimate that as many as one in five children have difficulties learning to read.  Reading has been associated as an early indicator of academic success.  Public formal education does not typically start until ages 5-6.  Before then, parents and caregivers are the first education providers during the 0-5 early critical years. The 1000 Books Before Kindergarten challenge is a simple (read a book, any book to your child, with the goal of reading 1,000 before kindergarten) and very manageable endeavor.
